Case management is designed to improve a client’s health, welfare, wellness, and autonomy. Case management is a pathway for providing accessibility to resources and unmet needs. We assist in helping people who are food, and housing insecure by providing linkage to resources including physical and mental health, financial, legal, spiritual, and other health and human services.
